Penis sleeves and extenders: how to choose the right cock sleeve

Updated October 12, 2026

A penis sleeve is a hollow, stretchy sheath that you pull over your erect cock. Depending on the model it adds length, girth, texture or vibration, but only while you wear it: a sleeve is a sex toy, not a treatment. Below are the main types, how to pick a size that stays on, which lube to use and what you can realistically expect.

What a cock sleeve is, and what it is not

Penis sleeve, cock sleeve, penis sheath, extension sleeve or extender: shops use all these names for the same family. They slide over the shaft, and most anchor behind the balls with a strap or ring.

A pump sleeve is something else: the soft seal at the opening of a penis pump, which keeps the vacuum in and is not worn during sex. Traction devices worn for hours a day are not covered here either.

The main types of penis sleeve

Open ribbed sleeves

An open sleeve leaves the tip free, so you keep direct feeling while ribs on the outside give your partner texture. The XR Brands Really Spacious ribbed sleeve has very thick ribbed walls and a separate hole for the balls.

Penis extension sleeves that add length

These have a closed, often solid tip in front of your own head. The Sono No.32 elastic extension adds 2.5 cm of length and a few millimetres of girth, a modest step that still feels natural. The XR Brands Penis Sleeve 4 cm sits in between, with a soft semi-solid tip and nubs on the inside as well as the outside. The XR Brands Penis Extension Sleeve 7.5 cm goes much further, with a solid tip, a realistic head and a base you can trim for a better fit. The more solid tip there is, the less you feel at the end of each thrust, so start modest.

Girth sleeves

When thickness matters more than length: the Sono No.36 thick extension adds the same 2.5 cm of length as the No.32 but about 1 cm more diameter. The Crushious Ribbed Penis Sleeve is soft and transparent with raised waves along the whole shaft, and the RealRock Penis Sheath 16 cm is made of a skin-like TPE with a realistic shape and a 4 cm diameter.

Sleeves with a ball loop

The ball strap is what keeps a sleeve on. The Doc Johnson Thin Extension with Ball Strap adds length while keeping the profile slim, a sensible first sleeve. The strap works a little like a cock ring, so the same rules apply: snug, never tight. Our cock ring guide explains fit and wearing time.

Vibrating sleeves

With a motor or bullet built in, both of you feel the buzz. The XR Brands Vibrating Textured Erection Sleeve is short, with a bullet in a ribbed pad at the base and a strap behind the balls. The XR Brands silicone vibrating sleeve with remote adds girth and has two motors, a remote and a USB rechargeable battery. For texture without any extra length, the Levelz Silicone Cockring and Ribbed Sleeve combines a firm ring with a short ribbed section.

Materials: TPE, TPR, silicone and "elastic"

  • TPE and TPR (thermoplastic elastomer and thermoplastic rubber) are very soft and stretchy, so one size fits a wide range of men. They are slightly porous, so replace a sleeve that turns sticky, cloudy or torn. Sleeves described as "elastic" or "stretchable" are nearly always TPE.
  • Silicone is firmer, stretches less, is non-porous and lasts longer. It is easier to keep hygienic, but the opening has to be closer to your own size.

How to choose size, length and girth

Measure while fully erect. Length runs along the top from the base to the tip; girth is the circumference at the thickest point, and girth divided by 3.14 gives your diameter. Our size calculator does the maths, and Size matters explains how to measure properly.

  • Opening: with TPE and TPR the unstretched opening can be clearly narrower than your shaft, because the material stretches. With silicone, choose an opening close to your own diameter.
  • Hollow length: compare the insertable length with your erect length. If you are longer than the hollow part, you press into the tip and lose some of the extra length; if you are much shorter, the tip wobbles. A centimetre or two either way is fine.
  • Extra girth: going from your own size to 1 cm more diameter is already a clear difference for a bottom. Start there before choosing the thickest sleeves.

Lube compatibility

Use a water-based lube with every sleeve. Silicone lube damages silicone sleeves, and oil, massage oil and petroleum jelly make TPE and TPR swell and break down. A few drops inside help the sleeve slide on; too much and it slips around. Put more on the outside for your partner and top up during sex, because a thick sleeve dries out faster than skin.

How to put on a penis sleeve

  • Start when you are at least half hard.
  • Put a few drops of lube inside the opening and a little on your shaft.
  • Stretch the opening with your fingers and slide or roll the sleeve on until the base sits against your body.
  • Pull the strap or ring behind your balls, one ball at a time.
  • Check the fit: snug, no pinching, normal skin colour.

Take it off after about 30 minutes, or straight away if you feel numbness, pain or coldness, or your skin turns blue or dark.

Using a cock sleeve with a partner

Talk about it first. Extra girth means a bottom needs more warm-up and more lube, while you feel less because there is material between you. Begin with a slim or modest sleeve, go slowly at first and let your partner set the pace. Texture on the outside is for your partner, nubs on the inside are for you, and vibration works for both.

Sleeve or extra thick condom?

People who look for extra thick condoms usually want to last longer or feel more secure. A thicker condom dulls sensation a little and still protects. A sleeve with a closed tip dulls sensation much more, but protects nothing: fluid can pass at the base and through open ends, and a regular condom will not roll over a thick, textured sleeve. If protection matters, use condoms on their own and keep a sleeve for partners you have safer sex agreements with.

Hygiene and storage

  • Wash it after every use with warm water and mild soap or a toy cleaner, inside and out.
  • Let it air dry completely before you put it away.
  • Store TPE and TPR sleeves on their own, away from other toys and sunlight.
  • One sleeve per person is the hygienic rule.

Honest expectations

A sleeve adds size only while you wear it. It does not make your penis longer or thicker afterwards, whatever a package suggests. You will feel less than with bare skin, especially with a solid tip, which helps many men last longer. The extra size is real for your partner, but it looks and feels like a toy rather than your own skin. Expect to try a size or two before one feels right. All models are in penis sleeves and extenders.

Frequently asked questions

Does a penis sleeve make your penis bigger permanently?

No. A sleeve adds length or girth only while you wear it. When you take it off, nothing about your body has changed.

Can I use silicone lube with a cock sleeve?

Not with a silicone sleeve, and to be safe not with TPE or TPR either. A water-based lube works with every material and washes off easily.

How long can I wear a penis extension sleeve?

Around 30 minutes at a time is a sensible limit. Because the ball strap works like a cock ring, take it off at once if you feel numbness, pain or cold.

Can I wear a condom over a penis sleeve?

A standard condom will not roll over a thick, textured sleeve and is likely to tear or slip. A sleeve does not protect against STIs, so if you need protection, use a condom without a sleeve.

Which sleeve should I start with?

A modest extension in soft TPE with a ball strap, one that adds about 2.5 cm and little girth. Go thicker or longer once you know how it feels.

Is a cock sleeve the same as a pump sleeve?

No. A pump sleeve is the soft seal at the opening of a penis pump. A cock sleeve is worn over your erect penis during sex.
